Attending Physician

  • Certifies the terminal prognosis
  • Assess patient needs, manages, & prescribe treatment
  • Directs & approves the plan of care
  • Coordinated care with interdisciplinary team

Social Worker

  • Assess patient & family with emotional. spiritual, & financial needs
  • Provide direct counseling or refer patients & families to appropriate community
  • Develop a plan of care to meet identified needs
  • Provide bereavement

Medical Director

  • Co-Certifies the terminal prognosis with the attending physician
  • Leads the interdisciplinary team in the development of the plan of the care
  • Provides consultation to other physicians regarding hospice care

Chaplain

  • Assess patient & family spiritual needs
  • Develop a plan of care to meet identified needs
  • Provide direct counseling
  • Assist with memorial preparations
  • Provide consultations to community clergy

Home Health Aide

  • Assess patient & family needs
  • Develop the plan of care to meet identified needs
  • Coordinate team visits & ensure implementation of approved plan of care
  • Coordinate care with attending physicians, primary caregiver, and/or Care Center Staff
  • Ensure symptom control & pain management
  • Provide patient/family teachings as needed

Volunteers

  • Provide companionship & support to patient & family
  • Provide needed non-medical services letter writing, errands, & respite items for family
  • Provides support at time of death & during bereavement

Registered Nurse

  • Assess patient & family needs
  • Develop the plan of care to meet identified needs
  • Coordinate team visits & ensure implementation of approved plan of care
  • Coordinate care with attending physicians, primary caregiver, and/or Care Center Staff
  • Ensure symptom control & pain management
  • Provide patient/family teachings as needed
